Set up a new CADD memorial Fund

Setting up a CADD Memorial Fund is a simple, three stage process. Simply fill in the details of the memorial fund and click the 'Next' button. You will then be taken to a page where you can send us photographs of the person. Then you will be taken to a donation page - you do not have to make a donation, but if you do, you will show as being the first donation in the memorial fund. That's all there is to it.

In a few days your fund will then be added to the list on the web site, and you will then be able to view the fund, make donations and direct friends and relatives to the page to make their donations. You will receive an e-mail when your fund has been set up, and if you then visit our Memorial Funds page you will see your new fund.

Now, please complete the following details as thoroughly as you wish - please note that the fields outlined in red must be filled in. When you have finished, click the 'Next Stage' button for the next stage. We would recommend that if you are entering a large amount of text for a tribute etc., you should enter this first into a word processor and copy and paste it from there to the appropriate field on the form, just in case it gets lost when moving to the next stage.
